Photo/Video Release: I hereby grant permission for the Gainesville First United Methodist Church to use photographs or video clips in which I or members of my immediate family appear for future informational or promotional events of the church (i.e. brochures, posters, websites, video productions, etc.)
Medical Release: I hereby grant permission for the adults in charge of this retreat to Woodlands in Cleveland to seek emergency treatment for my child by a licensed physician or hospital in the case of illness, accident, or injury, and grant them permission to release any information requested for the completion of medical/surgical or accident claims for my child. I further agree not to hold GFUMC, or any adult in charge thereof, responsible for any accident or illness that might occur to my child while attending this event sponsored by GFUMC.
